Locally owned and operated in-home Care company is seeking an EXPERIENCED  Client Case Manager to join their team.

Primary responsibilities include by are not limited to;
  • The Client Case Manager works together with the service team to support in home services for seniors, persons with disabilities, and developmentally delayed children and adults.
  • This position is responsible for creating, reviewing, tracking, and gathering information and create a follow up plan.
  • Educating clients on all services the company  provides and enroll them in the services/programs that best suit their needs.
  • Your role is to provide oversight of the plans of care, and to maintain client records in the database.

Bachelors in Human Services or related fields preferred but not required. 

This position is newly opened and interviewing for immediate hire (Pending Background Check).

Position is full Time Monday - Friday.

Salary range is $21 - $29  PH + Competitive Benefit
